I have made this thread because the format has been deleted. Obviously I don't remember word for word what the last format looked like but this is my application formatting. If anyone has adjustments please post underneath.
Would request a sticky once the format is perfected.

| Professional is simple and basic. The old format had like 5 questions that didn't tell you a whole lot about your capabilities. If he has anything that could add to this format i'd be happy to change mine, and it'd be easier to have one format instead of 1 new and 1 outdated one etc. | |
